Grab the vaccination card you received with your first dose and answer the following questions.
The information you provide will help us determine the correct vaccine for your next dose or booster, so please make sure your answers are right.

When referring to your COVID-19 vaccination record card for locating your information, it should include two sections. The first section has two lines for your personal information: Your last name, first name and middle initial are on the first line and your date of birth is on the second line. The second section includes vaccine manufacturer, dates and healthcare professional records added by the vaccinator for your COVID-19 first, second and follow-up vaccines.
If you have additional questions about your COVID-19 vaccination record card, please check with the location where you received your vaccine. They should be able to help you access your account and vaccine records.
